Alumni Chapter Registration Form Filing Instructions

You can fill the registration form manually or electronically and send it to the Treasurer for processing.

Manually you can print and fill-in the form or electronically you can edit the form using PDF Editor or fill-in the word formatted file, add the passport picture into the file, digitally sign and send it to the treasurer. Before you send this form to the Treasurer, please ensure filling the following mandatory items and follow the instructions on the form.

1) Form Mandatory items:

  •    COURSES STUDIED
  •    PLACE
  •    DATE
  •    SIGNATURE

2. Affix passport size photo in the form

3. Pay the membership fee as mentioned in the application form ($60) through any mode of payment as below:
